Medical Records

Medical Record Requests are handled electronically. If you are a third party organization, please complete the jotform below to submit a medical record request. We value the privacy of our clients information and cannot send client records without our company's Release of Information form completed by the client. Once our Release of Information forms and payment for records are received, records will be faxed within 10 business days. If you have any questions, please email medical.records@imanibehavioralhealth.com.

Invoices for medical records will be sent via email. For payment, we accept credit/debit cards. Below is the pricing for medical records:

Additional pages $0.50/page
